New Image has an Outreach Case Manager for adults 18 and older. The Outreach Case Manager visit places where homeless frequent (i.e.: parks, abandoned buildings and cars, encampments, etc.). The Outreach Worker's purpose is to educate homeless individuals on services that are available to them, such as: entitlements, housing opportunities and options, and also refer them to the Long Beach Multi-Service Center for free showers and laundry services, free medical, mail, transportation, etc..

Program Objectives

  1. Twenty-five (25) outreach homeless clients, or 23% of 108, will have been placed into housing, and have increased income through jobs or entitlements within three (3) months.
  2. Twenty-two (22) outreach homeless clients, or 20% of 108, placed into housing, will receive follow-up case management to assure success and longevity in their placement, for a minimum of six (6) months
  3. Thirty-three (33) or 30% of 108 outreach homeless clients will increase their income through mainstream entitlement programs,, or employment, within six (6) months of program entry.
